The Physical Plant Operations Report a Problem form collects personal information from Virginia Tech faculty and staff in order to facilitate maintenance and repairs. The information is emailed to a Physical Plant employee. No information is saved on the web server, so we do not share any personal information with any third parties.
The tent permit application form collects personal information from Virginia Tech faculty and staff in order to facilitate tent rentals. The information is emailed to a Physical Plant employee. No information is saved on the web server, so we do not share any personal information with any third parties.
Records Management Services uses two online forms to better serve their customers.

- Records Transfer Request
The information submitted by visitors who use the forms is emailed to Records Management Services and stored in log files. The data is used to fulfill requests on an internal Records Management system. The information may be also used for statistical analysis and reporting, including individual specific analysis and reporting for university use. Some results of such analysis and reporting may be posted to publically accessible web sites provided it is general in nature and not specific to one individual.
